Appointment Cancellation Policy
At Golden Health Acupuncture we understand that plans change and unforeseen things happen and we do our best to accommodate our clients in the case of cancellations. However, in order to allow for others to access our services we encourage that you let us know as soon as possible of any cancellations.
Please keep in mind the following:
- Cancellations with less than 24 hours notice will be charged 50% of the service price.
- Cancellations less than 1 hour notice will be charged 100% of the service price.
- All no-show appointments will be charged 100% of the service price.
Late Arrivals:
If you arrive late:
- your session may be shortened in order to accommodate others whose appointments follow yours.
- Depending upon how late you arrive, your practitioner will determine if there is enough time remaining to start a treatment.
Regardless of the length of the treatment actually given, you will be responsible for the amount of the “full” session.
COVID-19 Guidelines and Policies
At Golden Health Acupuncture we have put preventative measures in place to reduce the spread of COVID-19.
These measures include, but are not limited to:
- Air purifiers located in the treatment room and in common areas.
- Handsanitiser is provided in the treatment room.
- Staff and patients who have been exposed to Covid -19 or exhibit symptoms of illness are encouraged to stay at home.
- COVID-19 is contagious and patients must comply with all set guidelines to reduce the spread while attending appointments at Golden Health Acupuncture.
Notice of Patient Privacy
I understand that as part of my healthcare, Golden Health Acupuncture records and maintains health records describing my health history, symptoms, examination and test results, diagnoses, treatment and any plans for future care of treatment.
Any information that I choose to share with Golden Health Acupuncture, including my admission as a patient of this facility, will be kept confidential, used as specified below and shared only with my written consent.
I understand that this information serves as:
- A basis for planning my care and treatment.
- A means of communication among the many healthcare professionals who contribute to my care.
I understand that I have the right:
- To withhold permission to the use of my health information for directory purposes.
- To request restrictions as to how my health information may be used or disclosed to carry out treatment – and that Golden Health Acupuncture is not required to agree to the restrictions requested.
- To revoke this consent in writing, except to the extent that Golden Health Acupuncture has already taken action in reliance thereupon. I will notify Golden Health Acupuncture in writing if I require information to be shared with anyone other than those directly involved with my care at Golden Health Acupuncture.
